The California Hospice Foundation is a 501(c)(3) charitable foundation. Donations to the Foundation are tax deductable.

Through your contributions, you become our partner in educating and caring for our community. California Hospice Foundation works to improve access to quality hospice services and educate patients, families and health care professionals about excellent end of life care.

Our administrative costs are very low (currently 4.8%). We will use nearly all of your donation to improve access to quality hospice and palliative care services through research, education and public awareness.

Donation Form

We hope to be able to take on-line donations soon. In the meantime, we have a donation form that you can fill out on your computer or download to complete by hand. The form is in Adobe Acrobatİ Portable Document Format. (See below) You can also view the form as a web page.

Donations can be made in memory of or in honor of a loved one.

Payroll Deductions

Automatic deductions from your paycheck are an easy way to support hospice. Contributions can be made through the Combined Federal Campaign, California State Employees Charitable Campaign and Community Health Charities. You can also designate that your United Way contribution be given to the Foundation.

Planned Giving

Many people would like to make larger charitable gifts to support the organizations and institutions they care most about. But concerns about personal and family financial security may make these gifts seem impossible.

You might be surprised to discover that there are ways to fulfill charitable wishes, while actually enhancing your security or that of loved ones. Through their use, you may be able to increase the income from your property and/or arrange for management of specific assets. At the same time, income, estate and gift taxes may be avoided or minimized - all while completing the charitable gift you would like to make.
